Do answering services charge per appointment booked?

Some do. Smith.ai prints a $1.50 fee for each appointment its human receptionists book. Other live services fold scheduling into their minute rate, which means you pay for booking through longer calls rather than a line item. Always ask a live service two questions in writing: is there a per-booking or per-appointment fee, and is scheduling included on the plan you are quoting or sold as an upgrade.
