So look past the plan price to three things: whether Spanish is included or billed as an add-on, whether the service books the appointment in Spanish or just takes a message for someone to return later, and how the bill moves when your phone gets busy. The model is a US small business taking 80 calls in a normal month and 200 in a busy one, at a three minute average call (240 and 600 minutes). Those counts are our assumption, so rerun the math with your own phone bill. Three patterns stand out. The AI rows barely move between the two columns, because they bill flat, by call bundle or by unique caller. The live rows more than double, because a busy month is simply more minutes. And the Spanish column is where the surprises live: two providers charge for it per call, one charges a monthly amount it does not publish, and two say nothing about it on the page where you would look.
Do answering services charge extra for Spanish?
Some do and some do not, and you have to read each pricing page to find out. Among the twelve providers above, PATLive, Abby Connect, Moneypenny's minute plans, Rosie and PhoneAgent.ai include Spanish at no extra cost. Smith.ai lists a dedicated Spanish line at $1.00 a call, and AnswerConnect adds a monthly bilingual charge it does not publish.
A $1.00 per call Spanish charge sounds small and is not, once you do the arithmetic. A dental office or a contractor in Houston, Miami or Phoenix might take a third of its calls in Spanish. At 200 calls that is about 67 calls and $67 a month on top of the plan, or roughly $800 a year for something other providers include. With live services there is a second, quieter cost. The Spanish speaking operator pool is smaller, so the Spanish caller waits longer or gets a message taker when an English caller would have been booked.

Is there an AI receptionist that speaks Spanish?
Yes. Several AI receptionists answer in Spanish, and the better ones detect the caller's language from the first sentence and run the whole call in it, including the booking and the confirmation text. Language support is not the same as booking support. The test that matters is whether a Spanish speaking caller who wants an appointment on Thursday ends the call with an appointment on Thursday. Is a bilingual AI receptionist as good as a bilingual person?
For booking, hours, prices, directions and routing, a bilingual AI receptionist does the job as well as a person and answers faster, at any hour, several calls at once. For a caller who is upset, confused or describing something sensitive, a fluent human is still better, and the right setup sends those calls to one.
